Registration and Consent Form: Children and Young People’s Groups

You can complete this at your convenience from today. Alternatively, we will have QR codes available each Sunday morning between the first and second services, for parents who wish to enter information onsite.

Step by Step guide
When you click on the link above it will take you to a page asking for your email.
Please enter the email you have signed up to Church Center with, if you haven’t yet signed yourself up don’t worry enter the email you want to use to sign up. Once you have entered your email Church Center will send you a six-digit verification code.
Once verified it will take you into the form and ask you which member of your family you want to fill it in for. If your child isn’t on there yet you can click the button below that says add child you can then add their name and fill in the form for that child.
You will need to complete a separate form for each individual child so if you have three children you will need to complete three separate forms.
